PAC EXPENSE REIMBURSEMENT/REQUISITION (SY 2026-2027)

Reimbursement/requisition form

Reimbursements will be made only for items that have been pre-approved. 

Parents: Complete this form within 30 days of incurring expense.

Teachers: Submission deadlines are:

• December 11 - for expenses incurred between July 1 - December 11

• March 5 - for expenses incurred between December 12 - March 5

• June 4 - for expenses incurred between March 6 -June 4
